Food cravings in women with polycystic ovary syndrome (PCOS) may not be a matter of willpower, but rather a biological response, according to a study published in the European Journal of Endocrinology. This study explored appetite-related hormone responses and food cravings, finding distinct differences in how women with PCOS regulate hunger and fullness compared to healthy controls.

PCOS Linked to Altered Appetite Hormones

Researchers recruited 36 women with PCOS and 36 healthy participants. Women with PCOS had consistently higher food-craving scores. Fasting hormone levels in the PCOS group showed lower concentrations of oxytocin, GLP-1, and GIP (glucose-dependent insulinotropic polypeptide) compared to the control group.

After eating, differences became more pronounced. Oxytocin levels in healthy participants dropped early, corresponding with reduced hunger and increased feelings of fullness. Women with PCOS did not experience this early oxytocin response. The PCOS group also maintained lower GLP-1 and GIP levels before and after the mixed meal test.

Irmak Gunesli and colleagues noted that women with PCOS show lower fasting oxytocin and incretin levels alongside impaired postprandial responses. The PCOS participants showed altered communication between these appetite-related signals.

The Hormone Differences and Their Impact

Oxytocin, known for its role in social bonding, also plays a part in appetite regulation. In healthy participants, early changes in oxytocin aligned with reduced hunger and greater satiety. The lack of this pattern in the PCOS group may contribute to persistent cravings even after eating.

GLP-1 and GIP are incretin hormones released by the gut in response to food intake. They stimulate insulin production and influence feelings of satiety. Lower levels of both hormones before eating and blunted responses afterward could make it harder for women with PCOS to feel appropriately full, potentially driving repeated thoughts about specific foods.

The findings suggest that frequent thoughts about particular foods may be more than a simple preference or lack of discipline. Changes in hunger and fullness signals appear involved, reframing how healthcare providers approach conversations about eating habits with PCOS patients.
